Abstract

Background Over the past three years we have developed a simulation programme for our ODP students covering Malignant Hyperthermia (MH), LA Toxicity, and CICV. Due to a high demand from qualified Theatre and Recovery staff we have also ran refresher and update training sessions in situ during audit. We would like to describe the MH training session. Summary of education programme The teaching session utilises a PowerPoint presentation followed by a simulated MH scenario, which is run in-situ in an operating theatre. The consumables and equipment are the same that would be used in an actual MH crisis. During the initial couple of MH simulation sessions and subsequent debriefs, the overwhelming consensus from the participants was that, even though the AAGBI MH Guidelines are clear, concise and comprehensive it was difficult to keep track of the steps and equipment required and that it would be useful if a checklist was included with the crisis sheet. We had noted that the paper copy of the AAGBI guidelines was being annotated by the staff during the simulation. Using these notes we then developed our own specific checklist with the following brief, it had to be simple yet concise, and that it could be read out by anyone regardless of experience or role. Results In the sessions that followed the introduction of the checklist, we noted that the scenarios ran more effectively, roles were clearly defined and that all equipment that was required arrived promptly and timings were accurately recorded. Discussion The feedback from subsequent sessions using the new checklist showed staff felt more confident in using it and verbalising the steps alongside the AAGBI guidelines. This was found to be mainly because of its simplicity and similarity to the WHO surgical checklist. Conclusions and recommendations We now attach to the MH check list to the AAGBI MH guidelines on our MH trolley, so that they can be used in conjunction with one another. We feel that the checklist may also prove useful in the patients notes as a record of the Dantrolene doses and timings. We are continuing to run the simulated scenarios for all theatre staff.
