Abstract

Foundations of Clinical Pharmacy Practice is an academic book that provides Pharm.D learners with the necessary knowledge to enhance their diversity of understanding about the fundamental principles, activities, and the changing role of clinical pharmacists in medical care. The book recognizes the process of moving towards traditional dispensing to the patient-centered approach with focus on the evidence-based pharmacotherapy, appropriately managed medication therapies, adverse drug reactions, and effective patient counseling.
