Abstract

India's healthcare system is undergoing a significant transformation with the emergence of smart hospitals, driven by the integration of Internet of Things (IoT), Artificial Intelligence (AI), and automation. These technologies are enhancing patient care, streamlining clinical operations, and addressing long-standing challenges such as overcrowded wards, limited skilled personnel, and urban-rural healthcare disparities. This narrative review explores the current landscape and future prospects of smart hospitals in India, highlighting key implementations, enabling technologies, and barriers to widespread adoption. IoT-enabled solutions such as smart beds, continuous vital sign monitoring, and RFID-based asset tracking facilitate real-time data collection, remote monitoring, and automated clinical alerts. AI-driven tools support diagnostics in radiology and pathology, while robotic process automation (RPA) improves efficiency in scheduling, billing, and data management. Institutions like AIIMS Delhi, Narayana Health, and Apollo Hospitals have implemented these technologies to reduce hospital-acquired infections, optimize emergency resource allocation, and improve diagnostic accuracy. Government initiatives such as the Ayushman Bharat Digital Mission (ABDM) are promoting digital integration and interoperability across India's healthcare system. However, challenges persist, including high implementation costs, inadequate infrastructure in smaller cities, data privacy concerns, and limited digital literacy among healthcare workers. To fully harness the potential of smart hospitals, India must invest in public-private partnerships, capacity building, and robust regulatory frameworks. Smart hospitals hold the potential to build a future-ready, efficient, and inclusive healthcare system in India.
