Abstract
Among the main indicators of quality and Patient Safety in radiology there are the procedure errors in radiological exams. Patient safety errors on radiological imaging reports are frequently being caused by a failure to acknowledge of medical doctors and technician acts. X-rays are requested by medical doctor that relies on a report and image usually generated by a radiologist or radiographer. The report and image are sent to the referring health professional, who then acts on the result/diagnosis. The system for requesting radiology images and sending reports to the referring health professional is unreliable and has been proven to fail.
