Abstract

Sir, OpenAI is an artificial intelligence (AI) research and deployment company founded in 2015 with its headquarters at SFO, California. Its mission is to ensure that artificial general intelligence benefits all of humanity.[1] ChatGPT (generative pre-trained transformer) is a language model developed by OpenAI. As a language model, ChatGPT is designed to interact conversationally based on the input it receives. ChatGPT can have tremendous applications in the field of medicine. With its comprehensible language, it can be very beneficial for the general population to get a gist of any of the diseases/disorders. It is of interest to find out how ChatGPT would inform and guide the general population about a serious condition say for example cerebrovascular accident (CVA) commonly referred as “ischemic stroke” which requires prompt action [Table 1]. Ischemic stroke causes regional ischemia to the brain causing focal neurologic deficits lasting for more than 24 hours. Subtypes include thrombotic stroke (due to rupture of an atherosclerotic plaque), embolic stroke, and lacunar stroke (secondary to hyaline arteriosclerosis, a complication of hypertension).[2]Table 1: Chat on ischemic stroke between user and ChatGPTSo, what does ChatGPT say about ischemic stroke? Firstly, let’s talk about the positives. To begin with, ChatGPT explains the pathophysiology of ischemic stroke with clarity using a simple language that is easy for laypersons to understand. It says that “Ischemic stroke is a medical condition characterized by the sudden loss of blood flow to a part of the brain, leading to damage or death of brain cells.” Secondly, it states that “it is crucial for stroke patients to receive prompt medical attention and be assessed by healthcare professionals for appropriate management,” thereby urging the reader to get act promptly and raising awareness regarding the urgency of ischemic stroke. Thirdly, it gives all possible options for management in a concise manner. If we look at the negatives, the first thing that comes in mind would be that ChatGPT doesn’t provide any references which would raise a question regarding the authenticity of the data. Second, it doesn’t state the most important and common risk factor for ischemic stroke, i.e., hypertension—a fact that the entire general population should be aware of. Third, it doesn’t mention the time interval (4.5 hours from symptom onset) for which the candidate would be eligible for thrombolytic therapy. This vital information could change the prognosis for patients as it would prompt them more to act in a timely manner. There are reasons to believe that in the coming years, ChatGPT has the potential to substitute Google, where Google on the one hand provides hundreds of articles regarding a topic, ChatGPT on the other hand gives brief, to-the-point straightforward answers in simple short paragraphs. But like a coin with two sides, ChatGPT also has its pros and cons. It can benefit patients in a way that they would be able to acquire essential information on their own without relying completely on overworking physicians who are often not able to provide a substantial amount of their time for individual patient care. However, the question about the reliability of data needs to be addressed by providing references and using verified sources like World Health Organization (WHO). Global quality and reliability scores can be used to grade the data. Moreover, over-reliance and dependence on AI should not occur to such an extent that the general public start self-diagnosing and self-medicating their condition without having the required medical knowledge.
