Synthetic health insurance claim fakes: AI-generated injury videos inflate claims and burden chronic disease patients

Abstract

Dear Editor, Generative artificial intelligence (AI) developments have made it possible to produce synthetic videos that are very realistic and show injuries, thereby making it easier to commit fraudulent health insurance claims. Deepfakes are one such technology that alters audio, video, and images to create false proof of harm, and they are being more and more used to claim reimbursements for treatments, diagnostics, and rehabilitation services. This new danger adds to the healthcare fraud that is already present, which is estimated to take up 3–10% of total spending, translating into $100–170 billion a year in the U.S.[1]. AI-generated videos of injuries are a source of fraud as they create convincing visual evidence, such as staged accidents or symptoms that are exaggerated, and hence, the whole process is being made much easier by eliminating traditional authentication problems. The recent enforcement actions are a clear indication of the magnitude: the 2025 National Health Care Fraud Takedown arrested 324 persons, including 96 licensed professionals for causing over $14.6 billion in intended losses, which is considered the largest in the U.S. history[2]. Reviews of literature confirm that machine learning is able to detect provider and patient fraud in claims data; however, the lack of labeled examples and inconsistencies in the data have been the main obstacles to progress. Both unsupervised and supervised models can spot patterns in 3–10% of the fraudulent expenditures, while the use of deepfake technology is still unexamined. The total amount of fraud globally is around $260 billion per year, with synthetic media being one of the main factors that cause the rise of low-value claims[3]. Insurer’s costs are inflated by this fraud, which in turn directly raises the premiums and out-of-pocket expenses for chronic disease patients who are managing pulmonary hypertension, diabetes, or cardiovascular conditions. Although deep learning models for fake image detection have shown promising results with high accuracy in discriminating between real and synthetic medical visuals, their use is still limited because of privacy concerns. If no action is taken, synthetic claims may double the fraud rates; thus making the resources meant for the rightful patients go to waste[4]. It is essential for health systems to require AI forensics to be used in claims processing, to incorporate blockchain for the purpose of traceability of evidence, and to put in place regulatory standards for the revelation of deepfakes. Also, journal editors and lawmakers have to make peer-reviewed trials on counter-AI tools their top priority. The adoption of these measures will be urgent in order to preserve premiums and ensure access for at-risk patients. This letter to the editor adheres to the Transparency in the Reporting of Artificial Intelligence in Research (TITAN) guideline[5].
